冠脉造影对无创性检查方法诊断冠心病价值的评估 被引量:2

摘要 目的 :利用冠状动脉造影检查 ( CAG)评价 5种无创检查方法对冠心病的诊断价值。方法 :对 65例患者CAG前 1周内 ,均行心电图 ( ECG)、动态心电图 ( Holter)、活动平板运动试验 ( TET)、超声心动图 ( UCG)、核素心肌断层显像 ( ECT)检查 ,将各项检查结果进行对比分析。结果 :ECT、TET、UCG、Holter、ECG诊断冠心病的敏感性分别为 90 .0 %、80 .0 %、70 .0 %、62 .5 %、5 5 .0 % ,特异性分别为 60 .0 %、88.0 %、72 .0 %、60 .0 %、64.0 % ,准确率分别为 78.5 %、83.1 %、70 .1 %、61 .5 %、5 8.5 %。结论 :无创检查方法中 ECT检查的敏感性最高 ,TET特异性较高 ,多种无创性检查方法相结合可提高冠心病的检出率和准确性。 Objective:To evaluate the value of 5 non-invasive procedures in the diagnosis of coronary heart disease (CHD) by coronary angiography (CAG). Methods:Conventional electrocardiogram (ECG), ambulatory electrocardiogram (Holter), treadmill test (TET), ultrasonic cardiography (UCG) and myocardial single photon emission computed tomography (ECT) were performed in 65 patients one week before CAG, and related findings were comparatively analyzed. Results:The sensitivity of ECT,TET,UCG, Holter and ECG in the diagnosis of CHD were 90.0%,80.0%,70.0%,62.5% and 55.0%; while the specificity were 60.0%, 88.0%, 72.0%, 60.0% and 64.0% with veracity rate 78.5%, 83.1%, 70.1%, 61.5% and 58.5% respectively. Conclusion:Of all the non-invasive procedures, ECT was most sensitive and TET more specific. Combination of multiple non-invasive procedures may improve the positive rate and veracity in the diagnosis of CHD.
