微创钢板固定技术在胫骨近端粉碎性骨折中的应用

摘要 目的 探讨微创钢板固定技术在胫骨近端粉碎性骨折的临床疗效。方法 选择 2 4例病例 ,通过各种方法复位后 ,于骨折两端胫前内侧做 2~ 3cm切口 ,部份胫骨近端选用胫前外侧切口。骨折近端选用高尔夫球棒型解剖钢板或LISS钢板 ,透视下置入 ,骨折两端拧入 3枚螺钉。结果 本组 2 4例经 3~ 2 6个月随访 ,平均 14个月 ,全部Ⅰ期愈合。手术时间 4 5~ 16 0min ,平均 80min。按Johner -Wruhs方法评价 ,优 15例 ,良好 7例 ,一般 2例。总满意率 94 1%。结论 微创钢板内固定对治疗胫骨近端粉碎性骨折成功率高 ,无并发症 。 Objective To discuss the curative effect of application of micro-invasive steel plate technique in treatment of splintered fracture of proximal tibia. Methods 24 cases were fixed with varied reduction, small incision (2~3 cm) was made at tibial fracture and golf stick steel or LISS steel was fixed with 3 screws under fluoroscopy. Results All 24 cases were heal in first stage after 3~26 months follow-up (14 months in average). The operation time was 45~160 min (80 in average). There were 15 cases excellent, 7 cases good and 2 cases healed with standard of Johner-wruhs method with total satisfactory rate 94.1%. Conclusion There might be a higher successful rate and easy to perform without complication for the micro-invasive steel plate inn treatment of of splintered fracture of proximal tibia.
