摘要 目的 研究喉癌组织中环氧化酶2(COX 2)的表达与临床病理参数及VEGF、MVD的关系,并探讨其临床价值。方法 采用免疫组化技术检测40例喉癌组织中COX 2、VEGF的表达情况,用CD34标记新生血管内皮细胞,在显微镜下观察微血管密度。结果 喉癌组织中COX 2、VEGF的阳性表达率分别为67.5% (27/40)和80% (32/40),MVD平均为52.46±16.96条/200倍视野。与声带息肉及癌周正常组织相比,肿瘤组织中COX 2、VEGF表达及MVD明显增加,COX 2表达与肿瘤临床分期有关。COX 2表达与VEGF有显著相关性,且COX 2、VEGF表达阳性者MVD明显增加。结论 喉癌组织中存在COX 2的高表达。COX 2与肿瘤新生血管形成有关,它可通过增加VEGF的表达促进肿瘤血管形成,从而促进喉癌的生长和转移。 Objective To approach on the relationship of the expression of cyclooxygenase-2 linked with several clinical pathological characters and vascular endothelial growth factor(VEGF) expression and microvessel density (MVD) in laryngeal carcinoma. Methods The representatives paraffin section were selected and 40 specimens of laryngeal carcinoma were tested by immunohistochemical technique with polyclonal antibody against COX-2 and VEGF CD34 was used to detect vascular endothelial. MVD was determined by microscopy. Results The positive rates of COX-2 and VEGF expressions in laryngeal carcinoma were 67.5% (27/40)and 80%(32/40) respectively and the mean value of MVD was 52.46±16.96. The expression of COX-2 was higher in laryngeal carcinoma than that of in vocal cord polyp and in paratorgue cancer tissues( P <0.01). The group with high TNM stage had a significantly higher expression of COX-2 than low TNM stage group( P <0.01).The expression of COX-2 was significantly correlated with VEGF( P <0.01). Increased expressions of COX-2 and VEGF were significantly correlated with an increased in MVD ( P <0.01).Conclusion COX-2 overexpression is found in laryngeal carcinoma. It play an essential role in tumor growth invasion and metastasis. COX-2 may stimulated angiogenesis through VEGF upregulation.
