针刺取穴与骨折愈合疗效关系初探

Preliminary Approach to Relationship between Acupoint Selection and the Curative Effect of Acupuncture Promoting Fracture Union

目的 :初步探讨针刺取穴与骨折愈合疗效的关系 ,为临床研究和应用提供参考 ,方法选用 48只体重 2 .0～ 2 .5 kg的雄性纯种大耳白家兔 ,手术造成新鲜桡骨骨折模型 ,用区组随机法分为局部和邻近取穴组(A组 )、远道和辨证取穴组 (B组 )、局部和邻近取穴加远道和辨证取穴组 (C组 )、空白对照组 (D组 )共四组 ,术后第一天开始对治疗组分别做电针治疗 ,于术后 7d、1 4d、2 8d取材做血液生化学、生物力学、放射学、组织学的观测。比较各组间上述指标的差别。结果 :血清碱性磷酸酶含量 1 4d时 A组和 C组明显高于 B组和 D组(P<0 .0 1 ) ,骨折标本抗弯强度在 2 8d时 A组和 C组明显高于 B组和 D组 (P<0 .0 1 ) ,X线量化评分 1 4d和2 8d时 A组和 C组明显高于 B组和 D组 (P<0 .0 5 ) ,在各个时段各个指标 A组与 C组、B组与 D组比较均无显著差异 (P>0 .0 5 )。结论 :局部和邻近取穴能明显促进骨折愈合 ,而且具有操作简便 ,易于推广等优点 ,远道和辨证取穴则效果不明显 ,与前者合用也没有明显增强前者的疗效。

Objective:To preliminary approach to relationship between acupoint selection and the curative effect of acupuncture promoting fracture union,provide reference to the clinical study and application.Methods:48 male pure DaErBai rabbits weighted 2.0～2.5 kg were selected.then form the 1 mm bone-deficiency of new fracture model at both radial bone by operation.Devide into four groups randomly:local and neighboring acupoint group (A group),distancing and BianZheng acupoint group (B group), all acupoints of A group and B group (C group),control group (D group).Electro-acupuncture started at the intraday after surgery.7 days?14 days?28 days after experiment,the levels of alkaline phosphates (ALP) in serum?Biomechanic of bone?X-ray of the bone?Histology of bone were measured.Then compare the results of all groups.Results:At 14th days,the level of ALP in serum of A group and C group are higher then B group and D group,have significant difference (P<0.01).At 28th days,the bone mechanical intension of A group and C group are higher then B group and D group,have significant difference (P<0.01).At 14th days and 28th days,the X-ray score of A group and C gruop are higher then B group and D group,have significant difference (P<0.05).But the difference between A group and D group is not significant about all result in different period (P>0.05),the same as B group and D group.Conclusions:Stimulate local and neighboring acupoint can promote fracture healing effectively.and it can be handy operated and easily spreaded.Distancing and BianZheng acupoint can not promote fracture healing effectively,and it can not build up the effect of local and neighboringacupoint group significantly.
