^(18)F-FDG-PET成像技术在肿瘤诊疗方面的应用 被引量:1

摘要 正电子发射型计算机断层显像(positron emission tomography,PET )是核医学最高水平的标志,PET技术是可显示生物物质相应活动的空间分布、数量及其随时间的变化,故又称生物现象或分子显像,其最常用的显像剂为带正电子的放射性核素,如11C、13N、15O、18F等,以18F-FDG 为示踪剂的PET影像以广泛应用于肿瘤临床,使肿瘤的诊断、分期和治疗方案的选择发生了根本性的变革.大量的研究表明,在肿瘤原发疾病的诊断、转移灶显示、肿瘤分期、疗效监测及肿瘤复发与瘢痕鉴别方面的临床价值较大[1].18F-FDG 是葡萄糖的同分异构体,它进入肿瘤细胞内在己糖激酶作用下分解为18F-FDG-6-PO4, 无法继续完成糖代谢同时也难以回到细胞内,形成肿瘤细胞内的大量集聚.肿瘤细胞内糖酵解增强,葡萄糖代谢活性显著高于正常细胞,因而对18F-FDG摄取增高.PET能提供生理信息,使恶性肿瘤的诊断依靠改变组织的新陈代谢.计算FDG的利用率,常用suv 值定性肿瘤及表示恶性程度,阈值suv>2.5常作为恶性病变的判定标准.
