眶上额外侧锁孔入路治疗前颅窝底和鞍区肿瘤 被引量:4

Surgical Experience with Supraorbital Frontolateral Keyhole Approach for Tumors of the Anterior Skull Base and Sellae Region

摘要 目的探讨眶上额外侧锁孔入路切除前颅窝底和鞍区肿瘤的临床效果。方法对30例采用眶上额外侧锁孔入路和30例传统经额下入路治疗的前颅窝底和鞍区肿瘤病例进行前瞻性对照研究。结果眶上额外侧锁孔入路与传统额下入路相比,肿瘤切除程度、主要术后并发症和术后复发率无显著性差异。但是锁孔入路手术创伤小,术中出血少,术后平均住院日缩短。结论眶上额外侧锁孔入路治疗前颅窝底和鞍区肿瘤创伤小且疗效满意。 Objective The purpose of this study was to evalnate the results of the supraorbital frontolateral keyhole approach performed on tumors of the anterior skull baso and sellae region. Methods 30 patients with tumors of the anterior fossa and sellar region were operated upon us- ing this approach between January and September 2001. The surgical results were analyzed prospectively and compared with traditional subfrontal approach. Results We found no significant difference in the tumnor resection degree, main complications and tumor recurence ofter operation be- tween the two different approaches. But Patients with the supraorbital keyhole approach had less operative trauma, achieved good out - looking and shorter hospitalization. Conclusion In our experience, the supraorbital keyhole approach with a eyebrow skin incision is a safe and convenient minimal-invasive approach for experienced neurosurgeons in the treatment of lesions of the sellar region.
