异体脱钙骨基质粉复合自体骨髓构建可注射人工骨的实验研究

摘要 目的:探讨异体脱钙骨基质粉(DBM)复合自体骨髓(AM)构建可注射人工骨替代材料的可行性,检验其异 位诱导成骨能力。方法:将24只新西兰兔随机分为3组,分别在背部皮下注射DBM/AM、DBM、AM,术后2,4周取 材,通过大体观察、X线、组织学检查、扫描电镜研究以及碱性磷酸酶(ALP)测定,进行比较观察。结果:DBM与 AM按照一定比例复合后可以通过注射途径应用,X线、组织学检查结果均表明DBM/AM组有较多新骨形成,新生 骨具有丰富的哈氏系统结构,骨基质内胶原纤维的结构排列交错紊乱,新生骨边缘可见大量的成骨细胞突起;DBM 组仅见微量软骨和骨新生,AM组诱导出少量新生骨。DBM/AM组ALP水平较DBM组、AM组高,统计学上具有显 著性差异(P<0.01)。结论:DBM/AM复合可注射人工骨具有良好的骨诱导性和应用价值。 Objective: To evaluate the feasibility and osteoinductive potential of an injectable bone substitute consisting of allogeneic demineralized bone matrix powder ( DBM ) and autogeneic marrow ( AM ) . Methods: Twenty-four rabbits were divided into 3 groups and composites of DBM/AM, DBM and AM were injected subcutanously in rabbits back respectively. Specimens were X-ray, evaluated histologically, examined under scanning electron microscope and analyzed for alkaline phosphatase at 2 and 4 weeks postimplantion. Results: All implants can be injected succeedly, among them implant of DBM/AM induced new bone effectively and contained significantly higher level of alkaline phosphatase compared with the others (P<0. 01). Furthermore, the new bone has rich Haversian system such as bone lacuna and bone canaliculus. But grafts of DBM and AM stimulated litter osteogenesis. Conclusion: Injectable composite of DBM/AM exhibitted excellent osteoinductive activity and might be a valuable bone substitute.
