大鼠急性脑缺血再灌注的MRI研究 被引量:3

摘要 目的研究弥散加权成像(DWI)及血流灌注成像(PWI)在大鼠大脑中动脉阻塞及再灌注过程中的动态变化以及病理学基础。材料和方法35只SD大白鼠随机分4组,A组(5只)假手术对照组,B、C、D组(各10只)行线栓法右大脑中动脉缺血(MCAO)模型制作后,再灌注前、再灌注后不同时间点行DWI、PWI、T1WI、T2WI扫描,将检查结果与TTC染色及病理检查比较。结果B组再灌注后DWI异常信号相对面积无明显变化(P>0.05),再灌注前后ADC值无明显改变(P>0.05)。C组再灌注7d时6只大鼠DWI见高信号,但ADC图未见异常。D组再灌注后24hDWI异常信号面积较前增大(P<0.05),再灌注前后ADC值无明显改变(P>0.05)。D组感兴趣区ADC值明显低于B组(P<0.05)。再灌注后B、C组CBF、CBV图基本恢复正常,MTT图显示局部血流时间延长,D组较B、C组血流恢复更不均匀、不完全,局部存在灌注缺损。结论DWI能反映急性脑缺血再灌注后脑损伤的变化,PWI能反映再灌注后脑组织血流灌注情况。 Purpose:Combined with pathological observation,to study the change of DWI,PWI in the occlusion and reperfusion of the right middle cerebral artery of rats.Materials and Methods:35SD rats were divided into four groups,group A(n=5)was sham-operated for control study,group B,C,D(n=10for each)experience the occlusion and reperfusion of the right middle cerebral artery with thread.DWI,PWI,T 1 WI and T 2 WI were performed at different time points before and after reperfusion.The results of seˉrial MRI were compared with TTC stain and pathological observation.Results:After reperfusion,there was no significant change in the abnormal signal area of DWI and the value of ADC in Group B(P>0.05).Seven days after blood reperfusion in Group C,DWI showed abnormal signal in six rats but ADC map showed no abnormal in all10cases.In Group D,twenty-four hours after reperfusion abnormal signal area of DWI was larger than before(P>0.05).The value of ADC in the region of view was obvious lower in Group D than in Group B(P<0.05).In Group B and C,after reperfusion,CBF,CBV map showed reˉcovery of perfusion,and MTT map showed local time delay of blood flow in some cases.The recovery of perfusion in Group D showed more inadequacy and more uneven,and perfusion deficit still can be seen in local place.Conclusion:DWI can reflect the changes of the ischemic lesion after reperfusion.PWI can monitor the changes of blood dynamics.
