摘要
目的 探讨较大面积皮神经营养血管蒂逆行岛状皮瓣发生静脉回流障碍的原因及处理方法。方法 总结 33例皮神经逆行皮瓣修复四肢皮肤缺损手术 ,其中腓肠神经皮瓣 18例、隐神经皮瓣 8例、桡神经浅支皮瓣 7例 ;吻合静脉 15例、未吻合静脉 18例。结果 吻合静脉的全部成活 ,未吻合静脉的出现水疱 13例 ,因静脉回流障碍而坏死 3例 ,失败率 9%。结论 将皮瓣内浅静脉与受区静脉吻合 。
Objective To explore the cause and solution of venous reflux disturbance in neurocutaneous vascular resupinated island flap of superl area.Methods Summarizing 33 cases prothesis with this flaps,including 18 flaps of sural nerve,8 flaps of saphenous nerve and 7 flaps of ficial branch of radial nerve.The veins of 15 cases were anastomosed.Results 15 cases of anastomosing vein all survived.13 cases of unanastomosing vein occured blister.3 cases necrosised because of venous refulx disturbance.The failure rate was 9%.Conclusion It is the best solution of the venous reflux disturbance that anastomosing the vein of flaps and the vein of receivable area.
