摘要
目的 评价APACHEⅡ量表对ICU病房中中风病人近期预后的判断作用。方法 回顾分析连续48例中风病人的APACHE Ⅱ分值及其单项参数对近期死亡的判断作用。结果 48例中,9例死亡,39例好转出院或行外科手术,存活者的APACHEⅡ平均分值是7.1,明显低于死亡组的17.3(P<0.001),6例APACHEⅡ分值>17者均死亡,而<=16的42例中仅有3例死亡。通过对APACHEⅡ组成成分的分析,提示GCS评分,体温和血白细胞三项指标与中风病人近期预后显著相关。结论APACHEⅡ量表能用于中风病人近期预后的判断。
Objective To evaluate the value of APACHE II in predicting early mortality in stroke patients in an intensive care unit. Methods Retrospective analysis was applied with Acute Physiology and Chronic Health Evaluation APACHE I score in a total of 48 consecutive cerebral ischemic and hemorrhagic stroke patients in an intensive care unit and its individual components for predicting subsequent death. Results Of 48 patients, 9 died and 39 survived to either discharge or surgical intervention. The mean admitting APACHE Ⅱ score(7.l) of survivors was significantly lower than that(17.3) of patients who died. All of the 6 patients with a score over 17 died, so did 3 in 42 cases with a score over and equal to 16. Analyzing the components of APACHE Ⅱ score, it was found that Glasgow Coma Scale score, temperature, and white blood cell count were significant predictors of early prognosis. Conclusion The APACHE Ⅱ score may be used to predict early prognosis in stroke patients in an intensive care unit
