100例健康成人心率变异性频域分析

The Frequency domain analysis of heart rate variability in 100 healthy adults

目的检测健康成人的心率变异性之频域值,为临床提供正常参考值。方法健康成人100名,从20岁起分10岁为一年龄组,每组男、女各10名。应用动态心电图仪记录24小时心电图,经快速傅立叶转换取得心率能谱图(HRPSG)及各频带的心率能谱密度(HRPSD),低频带(LF)为0.04～0.15 Hz,高频带(HF)为0.15～0.4 Hz,总频带(TP)为0～0.4 Hz。结果24小时 HRPSD 为LF=1380±170,HF=494±160,TP=2364±92 ms^2/Hz,LF/HF3.2±1.5。各频域指标测值50岁前各年龄组无显著差异,而与50岁后各年龄组相比,差异显著。40岁后年龄组 LF 和 LF/HF 男性高于女性,HF 男性低于女性,差异显著。24小时中,LF 昼高夜低,HF 却相反。结论各频域指标正常值在40岁后应考虑性别、50岁后应考虑年龄之差别。反映自主神经张力的 LF 和 HF 之 HRPSD 昼夜呈动态变化。

Objective To determine the normal values of the frequency domain of heart rate vari- ability(HRV)in healthy adults.Methods The heart rate power spectral graph and the heart rate power density were studied in 100 healthy adults with 24-hour ambulatory electrocardiography with fast Fourier transformation.All subjects were divided into 5 groups with 10 years apart and each of them had 10 males and 10 females.Three frequency domain parameters of HRV were computed by integrating over their fre- quency intervals:low frequency(LF,0.04～0.15 Hz),hight frequency(HF,0.15～0.4 Hz),total power (TP,0～0.4 Hz).Results The values of 24-hour heart rate power spectral density were as follows:LF= 1380±170,HF=494±160,TP=2364±92ms^2/Hz,LF/HF=3.2±1.5.No significant difference existed in the indices of frequency domain between thegroups <50 years old,but conspicuous difference was pre- sent when compared with the groups >50 years old.LF and LF/HF were higher and HF was lower in male than in female with >40 years old subjects,the difference was significant.LF was high in day time and low in night time and vice versa with HF.HF had marked negative correlation with LF and LF/HF. There was significant postitive correlation between the LF and the LF/HF.Conclusion The results indi- cate that referring to the normal distribution of frequency domain indices more than 50 years old ,the differ- enees between age and sex should be considered.HRPSD of LF and HF,reflecting autonomic nerre tone, display a circadian alteration.
