气囊助产应用于过期妊娠的产程探讨与护理

Discussion on the Nursing Care of Air-vesicle Odinopoeia in Post-term Pregnancy

目的 了解气囊助产对过期妊娠分娩及产程的影响，并提出应注意的护理问题。方法与结果 将１２０例过期妊娠产妇按初产程随机分为两组，６０例行宫颈及阴道扩张的第一产程、第二产程、全产程时间分别平均是８．６，１．０５及９．７１ｈ；与对照组（三个产程的时间分别平均为１４．３，１．８１及１７ｈ）比较，观察组产妇的三个产程时间均缩短。观察组的会阴裂伤率、难产发生率与对照组比较，经统计学分析有差别，均降低，且产后合并症减少。结论 该方法不仅可明显缩短产程，且降低会阴裂伤率、难产率及母儿合并症，适用于过期妊娠的计划分娩，是一种简便、安全的助产方法。文章提出了在过期妊娠计划分娩使用气囊助产要严密观察产程进展，加强产时监护，要做好心理护理等护理措施。

Objective To understand the effect of air-vesicle odinopoeia on delivery and labor stage of post-term pregnancy and raise some corresponding nursing care problems for attention. Methods and Results 120 patients at labor stage of post-term pregnancy were randomized into two groups. In 60 patients who underwent vaginal dilation,duration of the first,second labor stage and the whole labor was significantly shorter than those of the control (8. 6, 1. 05 and 9. 71 h versus 14. 3, 1. 81 and 17. 0 h,respectively). In addition, the ratio of perineal laceration,dystocia and postpartum morbidity was also lower. Conclusion Air-vesicle odinopoeia not only shortened the labor stage,but also re- duced the ratio of perineal laceration,dystocia and postpartum complications of both the mother and the newborn. The procedure is handy and safe. The article also puts forward some nursing care mea- sures for attention,including observing the labor stage closely,reinforcing surveillance of labor and intensifying psychological nursing
