摘要
目的 旨在评价MR在诊断肺动静脉畸形中的作用和价值。方法回顾性分析 6例肺动静脉畸形患者的MR表现 ,并与DSA比较 ,观察评价成像质量及显示病变血管的能力。结果MR及CE MRA显示病灶呈瘤样扩张的血管 ,有一条供血动脉和一条或数条引流静脉与肺门相连。对比增强肺动脉MR成像 (CE MRA)可弥补常规序列的不足 ,有助于显示病变全貌 ,图像质量可与肺动脉造影相媲美。结论 作为一种无创性评价大血管疾病的方法 ,在显示肺动静脉畸形方面 ,MR特别是CE MRA有可能在一定程度上取代DSA。
Objective To evaluate the effect of MRI in the diagnosis of pulmonary arteriovenous malformation (PAVM). Methods The MRI and CE MRA of 6 patients with PAVM who were confirmed by pulmonary artery (PA) angiography were analyzed retrospectively, the imaging quality and the ability of displaying abnormal vascular were also evaluated. Results MR and CE MRA showed PAVM were as abnormal angiectasis with a feeding artery and one or several draining veins which tethered to the hilar region. The shortage of conventional MR sequence may be remedied by contrast enhanced MR pulmonary angiography which could show the full view of PAVM including the lesion, feeding artery and draining vein. The image was as good as pulmonary angiography. Conclusion In the evaluation of PAVM, contrast enhanced MR pulmonary angiography could not only show the angiectasis and its feeding artery and draining vein but also evaluate blood dynamics and prognosis. It maybe replace PA DSA in a certain extent as a kind of noninvasive technique. [
