胰腺囊实性肿瘤8例临床病理学分析

Cystic and solid tumors of pancreas:A clinicopathologic study of eight cases

目的 观察胰腺囊实性肿瘤 (CSTP)的临床病理及免疫组化特点。方法 组织学HE、PAS法 ,免疫组化SP法。结果 8例CSTP中 ,女性 7例 ,男性 1例 ;年龄 19～ 32岁 ,平均 2 6岁。手术后均无复发 ;肿瘤平均直径 9cm ,有包膜、囊实性相间。镜检 :肿瘤由乳头和囊实性区混合而成 ,细胞大小形态较一致 ,核圆、卵圆 ,异形不明显 ,核分裂罕见。免疫组化 8例α1抗胰蛋白酶、VIMENTIN弥漫阳性 ,2例CK阳性 ,3例CgA、Syn局灶阳性 ;Insulin、EMA阴性。结论 胰腺囊实性肿瘤多发生于年轻女性 ,具有独特临床病理特点。预后好 ,应视为低度恶性肿瘤。

Objective To study the clinicopathologic and immunohistochemical characteristics of cystic and solid tumors of pancreas(CSTP).Methods Eight cases of CSTP were studied using histologic(HE and PAS),immunohistochemical(SP methods).Results There were seven female and one male patients aged from 19 to 32(mena age 26).Without recurrence after tumor resection.The mean diameter of tumors was night cm,all encapsulated.Histological exmination showed presence of solid sheets,pseudopapillary with uniformed population of cells.In all of the cases,the nuclear were oval,round without atypia,and rate mitotic figures.Immunohistochemistry,all cases expressed Vim,α 1-AT,two cases expressed CK,three cases expressed CgA and Syn.All cases were negative for insulin,EMA.Conclusion CSTP,preferentially to young women,are uncommon tumors.It has distinguishing clinicopatholoic feature,a high cure rate by surgical resection,so it is a tumor of low-grade malignant.
