耳声发射技术在重症监护患儿听力筛查中的应用

摘要 目的:评估应用瞬态诱发耳声发射(transientevokedotoacousticemis-sions,TEOAE)对重症监护新生患儿进行听力筛查的价值。方法:收集1995-05-01/1998-04-30入住新生儿重症监护病房的患儿617例,男383例,女234例。纳入宫内感染、头颈部多发畸形、早产儿、低体质量儿(<1500g)、高胆红素血症者。排除标准:患儿监护人不同参加此研究者。研究对象重症监护新生患儿用英国Otodynamic公司生产的ILO-92耳声发射仪进行测试,刺激声为疏波短声,声刺激强度73~86dBSPL,刺激方式为非线性模式。结果:共有617例进行听力筛查,10例诊断为单耳和双耳中重度感音性听力下降,15例诊断为传导性耳聋。结论:TEOAE是快速、简单、客观、无创、有效的听力筛查技术,有助于早期发现重症监护患儿先天性感音性耳聋和传导性耳聋。 AIM:To evaluate the feasibility of transient evoked otoacoustic emissions(TEOAE) on the hearing screening in the intensive care neonates. METHODS:Totally 617 ill neonates in the neonatal intensive care unit from May1st ,1995 to April 30th, 1998 were selected, including 383 males and 234 females.Patients with intrauterine infection and mass hair and deformity in the head and cervical part,prematurs,neonates with low body mass(< 1 500 g) and with hyperbilirubinemia were included.Exclusive criteria:guardians were not in this study.The research subjects were examined with ILO-92 otoacoustic emissions apparatus made by Otodynamic Company in Britain.The stimulating sound was rare wave with short sound,the sound stimulating intensity was 73-86 dBSPL,and the stimulating pattern was the non-linear model. RESULTS:Of the 617 participants,10 neonates were diagnosed to have unilateral and bilateral sensorineural hearing losses;15 were diagnosed to have conducted deafness. CONCLUSION:TEOAE is a quick, simple, objective, undamaged and effective auditory screening technique.It is helpful to detect early the inherent sensorineural hearing losses and conducted deafness in intensive care neonates.
