无功能胰岛细胞瘤诊断与治疗(附20例报告)

Diagnosis and treatment of 20 cases of nonfunctioning islet cell tumor

目的 探讨无功能胰岛细胞瘤的临床特征和诊治方法。方法 收集我院1990～2004 年收治的无功能性胰岛细胞瘤20例,对其临床表现、检查手段和治疗结果进行分析回顾行分析。结 果 B型超声、SCT、MRI检查能提供肿瘤的位置,定性诊断主要靠术中冰冻病理切片检查。20例均 行手术治疗,其中16例随诊4个月～15年,良性12例,恶性4例,其中2例随诊复发而诊断为恶性。 结论 无功能胰岛细胞瘤发病隐匿,术前诊断困难,SCT,MRI对定位定性诊断有一定价值;术中病 理诊断有助于确定选择术式,病人随访对确定良恶性有帮助。

Objective To study the diagnosis and treatment of nonfunctioning islet cell tumor.Methods The clinical data of 20 cases of nonfunctioning islet cell tumor from 1990 to 2004 in the First Affiliated Hospital of Dalian Medical University were retrospectively analyzed.Results B-ultrasound, SCT and MRI provided the location of the tumors and the diagnosis was made pathologically. All of 20 cases were subjected to surgical treatment, and among them, 16 were followed up from 4 months to 15 years. There were 12 cases of benign tumors with a good prognosis and 4 cases of malignant tumors, of which 2 case were diagnosed as malignant tumors by recurrence.Conclusion The nonfunctioning islet cell tumor is delitescenced and difficult to diagnose before operation. SCT and MRI are helpful to the diagnosis of location and nature. Perioperative pathological diagnosis is helpful to select the operative method. The follow-up is benefit to definite the benign or malignant tumors.
