奥曲肽抑制大鼠部分肝移植的早期肝细胞凋亡

Octreotide inhibiting the early apoptosis of hepatocytes in orthotopic partial liver transplantation

摘要  目的 探讨奥曲肽对大鼠部分肝移植的早期移植物细胞凋亡的影响。方法 取清洁级健康SD大鼠108只,随机分成3组,每组随机分成18对。A组:供、受者间采用原位全肝移植;B组:供、受者间采用原位部分肝移植;C组:供、受者间肝移植方法同 B组,受者无肝期腹部皮下注射奥曲肽。采用细胞凋亡原位末段标记法检测移植物细胞凋亡率;免疫组织化学方法检测 Bax蛋白表达率。结果 三组大鼠肝移植后 180 min 时,肝细胞凋亡率分别为(24.2±2.4)%、(31.8±4.7)%、(24.0±2.5)%, B组较A组明显增多(P<0.05); C组又较 B组明显减少(P<0.05), C组与 A组比较,差异无统计学意义(P>0.05);三组大鼠肝移植后 Bax蛋白的表达在 180 min时阳性细胞率分别为(28.7±3.1)%、(36.2±5.3)%、(30.0±2.4)%。B组较 A组明显增加(P<0.05),C组较 B组明显减少(P<0.05),C组与A组比较差异无统计学意义(P>0.05)。结论 奥曲肽可抑制部分肝移植早期移植物细胞凋亡,改善部分肝移植术后缺血再灌注损伤。 Objective To observe the influence of Octreotide on early apoptosis of hepatocytes in orthotopic partial liver transplantation. Methods 108 SD rats were randomly divided into three groups: orthotopic liver transplantation group (group A, n =36), orthotopic partial liver transplantation (group B, n =36), and orthotopic partial liver transplantation combined with Octreotide subcutaneously used in anhepatic phase (group C, n =36). The apoptosis of hepatocytes and the expression of Bax protein were detected by TUNEL method and immunohistochemistry (SP method) respectively. Results In 180 min after liver transplantation, apoptotic hepatocytes in group B were 31.8% ± 4.7% and increased significantly as compared with group A ( 24.2% ± 2.4% , P < 0.05 ). At the same times, apoptotic hepatocytes in group C were 24.0% ± 2.5% and decreased significantly as compared with group B ( P < 0.05 ). There was significant difference between group A and group C ( P > 0.05 ). The positive cells of the expression of Bax gene in group B were 36.2% ± 5.3% and greatly higher than those of group A ( 28.7% ± 3.1% , P < 0.05 ), while the positive cells of the expression of Bax gene in group C were 30.0% ± 2.4% and greatly lower than those of group B ( P < 0.05 ). No significant difference between group A and group C was found ( P > 0.05 ). Conclusion Octreotide can reduce early apoptosis of hepatocytes in OPLT and protect graft from ischemia/reperfusion injury in orthotopic partial liver transplantation.
