闭合复位交锁髓内钉固定治疗胫骨骨折

Closed reduction and interlocking intramedullary nailing for tibial fracture

目的 探讨闭合复位交锁髓内钉内固定治疗胫骨骨折的临床疗效。方法 采用跟骨骨牵引、闭合复位、带锁髓内钉固定方法治疗 30例胫骨骨折。结果 30例中 1例由于术后过早负重行走 ,出现断钉而再次手术后愈合 ;2例出现局部皮肤坏死行转移皮瓣修复后愈合。骨性愈合时间平均 7个月。结论 跟骨牵引、闭合复位、小切口顺行交锁髓内钉固定能有效治疗胫骨骨折 ,具有易复位、创伤小、恢复快的优点 ,但不适用于近关节的胫骨骨折。

Objective To investigate the clinical effect of closed reduction and interlocking intramedullary nailing for the treatment of tibial fracture.Methods 30 cases of tibial fracture were treated with calcaneal traction,closed reduction and interlocking intramedullary nailing.Results 29 cases got bony healing with 1 case undergoing revision because of nail breakage due to too early weight bearing; 2 got regional skin necrosis and healed after flap transposition coverage.The bony healing period was 7 months on average.Conclusion It is effective to treat tibial fracture with calcaneal traction,closed reduction and antigrade interlocking intramedullary nailing, which has the advantages of easy reduction,minimal invasion and rapid recovery.It is not suitable for articular adjacent fracture.
