钢板螺钉内固定在枕颈融合中的应用

Application of Internal Fixation with Plate and Screw in Occipital Cervical Fusion

目的 探讨钢板螺钉内固定在枕颈融合中应用的疗效。方法 1999年 3月至 2 0 0 3年 7月 ,应用钢板螺钉内固定枕颈融合治疗难以复位的寰枢椎脱位并脊髓压迫 11例 ,其中齿突陈旧性骨折并寰椎前脱位 7例 ,齿突发育不良并寰椎后脱位 4例。结果 寰枢椎脱位获得不同程度的复位 ,脊髓压迫解除 ,神经功能明显改善 ,枕颈部 3～ 5个月骨性融合。结论 对于难以复位的寰枢椎脱位应用钢板螺钉内固定枕颈融合术 ,不但固定较牢固可靠、简便 ,有利于植骨融合 ,而且还有一定的复位作用。

Objective To investigate the effect of applying int ernal fixation with plate and screw in occipital cervical fusion. Methods From March in 1999 to July 2003, 11 cases of difficult reduction of atlas and axis and pressing against spinal cord were treated successfully, applying internal fixation with plate and screw in occipital cervical fusion. In which 7 cases of dens fracture with dislocation and 4 cases of hypoplasia of dena with dislocation of atlas and axis. Results In all of the cases, the dislocation of the atlas and axis was reducted to some extent, the pressing against the spinal cord was removed, and the function of the nerves was distinctly improved. The occipital cervical fusion have been recovered within 3 to 5 months. Conculsion For the fracture and dislocation of atlas and axis reducted difficultly,internal fixation with plate and screw is not only effective, dependable, simple and advantageous to fusion, but also helpful in the reduction.
