高龄患者下颌牙列即刻负重种植的修复对策 被引量:1

The Study of Immediate Loading of Implant-supported Fixed Denture in Octogenarians: A case report

摘要 目的 探讨老年患者下颌牙列缺失后采用即刻负重种植技术的治疗方案的制定及临床效果。方法 对1例下颌牙列缺失的 82岁高龄患者保留上颌残根和不良固定修复体 ,下颌植入 4颗Xive种植体 ,同期接入Temp Base临时基台行复合树脂临时桥修复。术后 8个月 ,参照上颌义齿 ,完成短牙弓烤瓷固定桥修复体。结果 种植体植入后追踪 2 6个月 ,没有种植体失败。患者对临时义齿及终义齿的咀嚼效能及美学效果均非常满意。结论 对高龄患者采用即刻负重种植修复时应严格掌握适应证 ,同时还要具有前瞻性 ,必须考虑全面 ,兼顾高龄患者的生理和心理上的特殊性 ,综合设计。 Objective To evaluate the clinical results of immediate loading of implants in fully edentulous lower jaw of octogenarians. Methods An 82-year-old man received 4 Xive implants surgery in the adentulous lower jaw. Provisional prostheses were cemented into the TempBase abutments at the time of fixture placement. 8 months after implant placement, 11 unit porcelain-fused-to-metal prostheses were fabricated and fixed to implant abutments by screws. Results No fixture has been lost since the immediate restoration was delivered, and the X-ray showed that 4 implants had excellent bone integration during the 26 months follow-up. The provisional prostheses and PFM prostheses could supply highly masticatory efficiency. Conclusion There are many psychological and physilogical problems in octogenarians. The placement of immediate restoration have revealed predictable results in octogenarians. Therefore, it's very important to improve their masticatory function and oral health through scientific and effective design and accurate fabrication.
