摘要
目的 :检测大鼠随意型皮瓣组织中TNF α(肿瘤坏死因子 )和ICAM 1(细胞间粘附分子 1)的表达变化 ,并观察抗氧化剂N 乙酰半胱氨酸 (N acetylcysteine,NAC)对任意型皮瓣的保护作用。方法 :采用Wistar大鼠 ,随机分为两组 ,在其背部形成 7cm× 2cm任意型皮瓣 ,对照组术后即刻腹膜下注射生理盐水 ,实验组术后即刻腹膜下注射NAC ,通过图像分析技术分别检测两组皮瓣成活率 ;并取皮瓣组织作组织学检查 ;用免疫组织化学方法检测TNF α的表达和ICAM 1的表达。结果 :实验组皮瓣成活率明显高于对照组 ;组织学检查发现 ,实验组皮瓣组织结构的破坏较对照组轻 ,炎症细胞浸润数量较对照组少 ;实验组皮瓣组织中TNF α和ICAM 1的表达显著低于对照组。结论 :NAC可以提高大鼠背部任意型皮瓣成活率 ,其机制可能与下调TNF α和ICAM 1等炎症相关因子的表达水平有关。
Objective:To investigate the expression of TNF-α and ICAM-1 o n the random pattern skin flap for rats and the protective function of N-acetye cysteine(NAC).Method:A random pattern skin flap (7 cm×2 cm) wa s designed an d elevated on the back of a Wistar rat.An image analysis technology was used to evaluate the survival rate of the skin flap,optical microscope was uesed to de tect the histological changes,while a immunohistochemical method was used to te st the expression of TNF-α and ICAM-1 protein in the tissue.Results:The survival rate of the flap in the NAC group is sign ificantly enlarged in seven days postoperatively,compared with the control grou p.The neutrophil counts in the NAC group is decreased significantly when comp ared with the control group.The level of the expression of TNF-α and ICAM-1 i s decreased in NAC group,compared with the control group.Conclusion:NAC can enhance the survival rate of the flap and th e mechanism could be related with its function of inhibition on the expression o f TNF-α and ICAM-1 of NAC.
