眶上锁孔入路显微手术切除鞍区大型和巨大型肿瘤

Trans-supraorbital keyhole approach microsurgery for treatment of large and huge tumors in the sellar region

目的探讨经眶上锁孔显微手术切除鞍区大型、巨大型肿瘤的手术方法和技巧。方法对31例大型、巨大型鞍区肿瘤采用该法实施手术,其中垂体瘤17例,脑膜瘤5例,颅咽管瘤7例,其他2例。结果大部或部分切除5例,残余瘤体给予放疗。本组无死亡。随访4个月～2.2年,2例肿瘤复发。结论采用锁孔显微手术能够切除鞍区大型、巨大型肿瘤,效果良好,并发症少。

Objective To explore the trans-supraorbital keyhole approach microsurgical technique for treatment of the large or huge tumors in the sellar region. Methods In 32 patients who had undergone the trans-supraorbital keyhole approach microsurgery, 17 (53.1%) suffered from pituitary adenoma, 5 (15.6%) meningiomas, 7(21.9%) craniopharyngioma and 3 (9.4%) other diseases. Results 27 (84.4%) patients had total tumor removal, and 5 had subtotal or partial tumor removal with incomplete tumor following radiotherapy. The patients were followed-up for 4 months to 2.2 years without death occurred and tumor regrowth was found in 2 cases. Conclusion Trans-supraorbital keyhole approach microsurgery for treatment of large and huge tumors in the sellar region can provide stable curative effect with less complication.
