摘要
目的 比较同种异体软骨耳支架、自体肋软骨耳支架、Medpor耳支架和自体肋软骨Medpor耳基复合耳郭支架等四种不同耳郭支架材料 ,应用于全耳再造的效果。方法 应用耳后皮瓣和颞浅筋膜瓣包裹耳支架行全耳再造修复 ,术后根据手术效果和并发症出现情况 ,对近远期各支架的满意程度进行评估。结果 自 1994年至2 0 0 2年 ,临床应用 16 8例 ,随访 3个月至 8年 ,同种异体软骨耳支架、自体肋软骨耳支架、Medpor耳支架和自体肋软骨Medpor耳基复合耳郭支架的满意率 ,分别为 2 0 %、75 %、85 %和 10 0 %。 结论 全耳再造以自体肋软骨Medpor耳基复合耳郭支架效果最好 ,Medpor和自体肋软骨次之 ,异体肋软骨效果最不理想。
Objective To evaluate the respective effect of four different auricular framework materials, including heterogenous cartilage, autogenous costal cartilage, Medpor and Medpor compounded with autogenous costal cartilage, used in total ear reconstruction. Methods Both posterior auricular skin flap and temporal superficial fascial flap were applied in total ear reconstruction, and the satisfactory rate of framework was compared each other postoperatively according to patient′s operative effect and complications. Results One hundred and eighty-six cases had used these frameworks to repair or reconstruct auriculae during recent eight years. The follow-up data showed that the framework′s satisfactory rate was 20%, 75%, 85% and 100% in heterogenous cartilage, autogenous costal cartilage, Medpor and Medpor compound with autogenous costal cartilage respectively. Conclusion The compound auricular framework (Medpor compounded with autogenous costal cartilage) has the finest effect in otoplasty, while Medpor and autogenous costal cartilage take second place and heterogenous cartilage is the least effective of all.
