脑梗死患者纤维蛋白原、血液流变学动态变化与β-纤维蛋白原基因-455G/A多态性的关系 被引量:15

摘要 目的:了解β-纤维蛋白原基因-455G/A基因多态性与脑梗死的关系,探讨高纤维蛋白原和血液流变学异常是否为脑梗死发病的危险因素。方法:2003-03/2004-08广西医科大学第一附属医院神经内科收治111例脑梗死患者为脑梗死组,123例体检者为正常对照组,均排除高血压、糖尿病、心脏病、高龄(>80岁)、感染、近期(3个月以内)创伤和手术、肿瘤等因素。用PCR-RFLP分析法检测β-纤维蛋白原基因-455G/A多态性在脑梗死组和正常对照组的基因频率。结果:发现人群中存在β-纤维蛋白原基因-455G/A多态性,G/A等位基因频率为0.789/0.211。脑梗死组β-纤维蛋白原基因的A等位基因频率显著高于正常对照组,差异有显著性意义(P<0.05)。脑梗死患者动态变化中的血浆维蛋白原、血液黏度比正常组高(P<0.05)。结论:脑梗死患者β-纤维蛋白原基因-455G/A的多态性与脑梗死的遗传学基础有关,A等位基因是脑梗死的易感基因之一。脑梗死患者的维蛋白原浓度增高,进而导致血液黏度增加,是脑梗死发生发展的危险因素。 AIM:To comprehend the relation between polymorphism of beta fibrinogen gene 455G/A and cerebral infarction(CI) in patients,and study whether abnormal fibrinogen and hemorheology are the danger factors for episode of CI. METHODS:111 cases of CI patients selected from the Department of Neurology,First Affiliated Hospital of Guangxi Medical University from March 2003 to August 2004 were the CI group,and 123 health examination people chosen from the same hospital at the same time were the control group. The cases of hypertension, diabetes mellitus, cardiac disease,>80 years old,infection and trauma,operation and tumor during the latest three months were eliminated.Genotype frequencies of beta fibrinogen gene 455G/A polymorphism in CI group and control group were estimated with polymerase chain reaction restrictive fragment length polymorphism(PCR RFLP). RESULTS:The polymorphism of β fibrinogen gene 455G/A was detected in Chinese with the G/A allele frequencies 0.789/0.211.The A allele frequencies of β fibrinogen gene in CI group was significantly higher than that in control group, and there was significant difference(P< 0.05). Dynamic changes of plasm fibrinogen concentration and blood viscosity in CI group were higher than that in control group(P< 0.05). CONCLUSION:The polymorphism of beta fibrinogen gene 455G/A is associated with genetics of CI and A allele is a susceptible gene for CI.The increased serum fibrinogen concentration leads to the increase of blood viscosity which is the independent risky factors for CI occurrence and development.
