青年脑卒中患者脑血液动力学参数变化特征

Characteristics of cerebral vascular hemodynamic parameters in young stroke patients

摘要 目的:比较和分析青年脑卒中患者脑血管血液动力学参数(cerebralvas-cularhemodynamicparameter,CVHP)的变化特征。方法:选择东北、华北等地区基线调查时收集的经头颅CT或MRI确诊的45岁及以下的脑卒中患者进行危险因素调查及CVHP检测,并对检测结果进行积分,对照组为根据病例组的年龄和性别以1∶2的比例随机选自同一人群中的非脑卒中个体。分析两组人群CVHP及其积分的差异。结果:病例组脑血管功能平均积分为(69.15±31.42)分,对照组为(87.67±19.16)分,病例组的积分显著低于对照组(P<0.01)。病例组的平均血流量(Qmean)、平均血流速度(Vmean)、最大血流速度(Vmax)、最小血流速度(Vmin)和临界压与舒张之差(DP)显著低于对照组(P<0.01),而外周阻力(Rv)显著高于对照组(P<0.05)。CVHP指标变化幅度较大的分别为DP,Vmin,Qmean,Vmean,Vmax和Rv。结论:病例组患者的CVHP变化特征主要表现为血流速度减慢、外周阻力升高和脑血管功能积分降低。 AIM:To analyze and compare the characteristics of cerebral vascular hemodynamic parameters(CVHPs) in young stroke patients. METHODS:Participants under 45 years diagnosed as having stroke with CT or MRI were selected from a cohort population in the northeast and northern China.Risk factors were investigated and CVHPs were checked during base line investigation and an index named accumulative score was estimated according to the CVHPs. Two folds sample of controls without history of stroke were selected from the same population according to age and sex.Defference of CVHPs and its accumulative score between two groups were analyzed. RESULTS:Mean score of CVHPs(69.15±31.42) in young stroke patients was significantly lower than that in controls(87.67±19.16)(P< 0.01).The mean quantity of blood(Qmean), mean velocity(Vmean),maximal velocity(Vmax),minimum velocity(Vmin) and pressure difference(DP) in young stroke patients were significantly lower than those in controls(P< 0.01).While,peripheral resistance(Rv) in young stroke patients was significantly higher than that in controls(P< 0.05).The obvious changes of indexes in CVHP were DP,Vmin,Qmean,Vmean,Vmax,Rv. CONCLUSION:It seems that the decline in velocity and accumulative score of cerebral vessels function, the increase in Rv are the main characteristics in young stroke patients.
