目的:总结手术干预先天性髋关节脱臼取得的进展及发展趋势。资料来源:应用计算机检索CNKI1991-01/2004-06相关先天性髋关节脱臼的文章,检索词“先天性髋关节脱臼,手术”,限定文章语言种类为中文。资料选择:对资料进行初审,并查看每篇文献后的引文。纳入标准:文章所述内容应为先天性髋关节脱臼手术治疗的方法。排除标准:重复研究、综述、Mata分析类文章。未排除文章中资料是否应用了随机、对照和盲法。资料提炼:收集到106篇相关文献,20篇文献符合纳入标准。排除的86篇文章中61篇内容陈旧,25篇系重复研究。资料综合:20篇文献中7篇涉及Salter骨盆截骨术的改良,3篇涉及Pemberton手术的改良,2篇涉及联合手术治疗,3篇涉及关节置换术,4篇涉及髋臼成形术和造盖术,1篇涉及有限元分析方法。结论:先天性髋关节脱臼的手术治疗已经取得长足进步,不同术式各有优缺点,掌握好手术指征和适应证可提高手术效果和患者术后功能恢复。计算机技术的引入有助于提高手术质量。

OBJECTIVE:To summarize the advancement and developing tendency of the operativ e intervention for congenital dislocation of the hip(CDH). DATA SOURCES:A computer was used to search for the articles about CDH from Jan uary 1991 to June 2004 in CNKI by inputting the retrieval words“congenital disl ocation of hip,operation' in Chinese. STUDY SELECTION:All the articles were primarily looked through,followed by loo king over the quotations in each article.The included articles were those relate d to the therapies for CDH.The excluded articles were those about repetitive stu dy,review,Mata analysis.The data among these articles did not exclude random,con trol and blind methods. DATA EXTRACTION:A total of 106 related literatures were collected,among which 20 were in accordance with the inclusion criteria.Among the excluded 86 articles ,61 were dated and 25 repetitive. DATA SYSTHESIS:Of the selected 20 literatures,7 were related to the improvemen t of Salter's osteotomy of pelvis,3 referred to improvement of Pemberton operati on,2 were about combination of operations,3 were with regard to joint replacemen ts,4 referred to acetabuloplasty and 1 was involved in finite element analytical method. CONCLUSION:A great advancement has been made in the operation of CDH,and there are some advantages and shortcomings of all the operations.The key to improve o perative effect and promote the postoperative functional recovery is to master t he indication and adaptation of operations.The introduction of computer technolo gy is helpful to improve the quality of operations.
