银杏叶提取物对肾病综合征炎症细胞因子的影响

Effect of Extract of Ginkgo Biloba Leaf on Inflammatory Cytokines in Children w ith Nephrotic Syndrome

目的 探讨银杏叶提取物对原发性肾病综合征 (NS)患儿外周血单个核细胞 (PBMC)中炎症细胞因子的影响。方法 将我科 2 0 0 0年 10月～ 2 0 0 4年 10月收治的小儿NS 3 5例 ,随机采用泼尼松加银杏叶提取物和单用泼尼松两种方法治疗 ,共分为 3组 ,治疗前组 3 5例 ,未治疗的患儿 ;银杏叶组 2 0例 ,采用泼尼松加银杏叶提取物治疗 ;激素组 15例仅给强的松治疗 ,3 0d为1个疗程 ,疗程结束后观察疗效。分别于治疗前后观察临床和生化检验指标 ,同时应用酶联免疫吸附试验 (ELISA)方法检测PBMC培养上清液中IL 1、IL 6、IL 8和TNF α蛋白水平的变化。结果 治疗 1个月后 ,临床症状及血液生化指标等明显改善 ,治疗前组与银杏叶组比较有显著差异 (P均 <0 .0 1) ;银杏叶组明显优于激素组 ,两组有显著差异 (P <0 .0 1)。对 3组IL 1、IL 6、IL 8和TNF α变化进行比较 ,银杏叶组和激素组均较治疗前组明显降低 ,有显著性差异 (均P <0 .0 1) ;银杏叶组较激素组也降低 ,有显著性差异 (P <0 .0 1)。结论 银杏叶提取物对NS患儿外周血中多种炎症细胞因子有抑制作用 ,对NS治疗有明显疗效 ,可用于辅助激素治疗。

Objective To explore the effects of extract of ginkgo biloba leaf on inflammatory cytokine s that were secreted in peripheral blood mononuclear cell (PBMC) in children wit h primary nephritic syndrome(NS). Such as interleukin(IL)-1,IL-6,IL-8 and tumer necrosis factor-α(TNF-α).Methods Thirty-five children with NS who were hospitalized during the period from Octobe r 2000 to October 2004 were treated randomly with prednisone plus extract of ginkgo bilob a leaf and only with prednisone 2 kinds of different treatments. We divided al l patients into 3 groups.The prior treatment group, including 35 children wh o received the treatment. The ginkgo leaf group were treated with prednisone plu s extract of ginkgo biloba leaf, including 20 cases. The prednisone group were t reated only with prednisone,including 15 children.Thirty days as a course of tre atment, then observed the therapeutic effects. At the same time, detected the ch anges of protein level of IL-1,IL-6,IL-8 and TNF-α by ELISA in the supernata nt fluid of PBMC culture. All variable were observed before and after treatment. Results After 1 month treatment, clinical symptom and biochemical detection in blood wer e ameliorated in ginkgo leaf group compared with prednisone group, there were si gnificant differences between 2 groups (P<0.01). The changes of protein leve l of IL-1,IL-6,IL-8 and TNF-α were compared, ginkgo leaf group and prednisone g roup were both lower than those in prior treatment group(P<0.01).Ginkgo le af group were lower than those in prednisone group(P<0.01).Conclusions Extract of ginkgo biloba leaf can inhibit the secretion of many cytokines in PBM C of NS,and has significant therapeutic effects,it is worth to be used in clinic as an assistant treatment of steroid therapy in children with NS.
