非小细胞肺癌患者外周血CK19mRNA检测的临床意义 被引量:3

Clinical significance of detecting CK19mRNA in the peripheral blood in non-small cell lung cancer patients

摘要 目的探讨非小细胞肺癌(nonsmallcelllungcancer,NSCLC)患者外周血CK19mRNA检测的临床意义。方法运用巢式逆转录聚合酶链反应方法(nestedreversetranscriptionpolymerasechainreaction,NestedRTPCR)检测78例NSCLC患者和32例肺良性疾病患者外周血CK19mRNA的表达,另选取20例健康成年人外周血作为对照。结果NSCLC患者外周血CK19mRNA表达阳性率为692%,肺良性疾病患者为63%(χ2=36014,P<001),20例健康成年人无一表达。NSCLC患者外周血微转移与组织学类型、细胞分化程度及TNM分期均存在密切关系(P<005或P<001)。结论CKl9mRNA是一种诊断NSCLC外周血微转移的特异性标记物,NestedRTPCR方法是检测外周血CK19mRNA表达的灵敏可靠技术,NSCLC微转移的诊断对判断病程发展和指导临床治疗具有重要意义。 Objective To evaluate the clinical significance of detecting CK19mRNA in the peripheral blood of patients with non-small cell lung cancer(NSCLC).Methods The expression of CK19mRNA was detected by nested-reverse transcription polymerase chain reaction(Nested RT-PCR) in the peripheral blood from 78 NSCLC patients,with 32 benignpulmonary lesion patients and 20 healthy adults as control.Results The positive rate of CK19mRNA in the peripheral blood was 69.2% in NSCLC patients,and 6.3% in benign pulmonary disease patients(χ2=36.014,P<0.01).The expression of CK19mRNA was found in none of the 20 healthy adults.The micrometastases in the peripheral blood of patients with NSCLC was closely correlated with the pathological classification and cell differentiation and TNM stage(P<0.05 or P<0.01).Conclusions CK19mRNA is a specifical marker in diagnosing micrometastases in the peripheral blood of NSCLC patients,and Nested RT-PCR is a sensitive and reliable technique for detecting the CK19mRNA expression.The diagnosis of micrometastases of NSCLC patients has great significance in judging the development of the disease and guiding the clinical treatment for patients.
