应激性溃疡并大出血26例

Treatment for stress ulcer with bleeding:A report of 26 cases

目的探讨应激性溃疡并大出血的临床治疗。方法对1992～2002年,我院收治的26例应激性溃疡患者临床治疗进行回顾性分析。结果本组26例,治愈22例(85%),死亡4例(15%)。死亡原因多器官衰竭(MOF)3例(12%),失血性休克1例(4%)。结论应激性溃疡的治疗在于积极治疗原发病,有效的控制出血,同时加强对机体重要器官的保护。外科手术是应激性溃疡并大出血危急状态的惟一治疗手段。

Objective To evalute the clinical treatment of stress ulcer with bleeding.Methods From 1992 to 2002,twenty-six cases with stress ulcer with bleeding were reviewed retrospectively.Results For the twenty-six patients,the cure rate was 85%(22 cases),mortality was 15%(4 cases).The deaths were caused by multiple organ failure(MOF) 12%(3 cases) and vena cava disruption 4%(1 case).Conclusions For the treatment of stress ulcer with bleeding,great attention should be paid to the treatment of primary disease,active control of bleeding and the protection of important organ function such as heart,brain and kidney.Surgical operation is the only important meathered in the treatment of stress ulcer with bleeding.
