矽肺结核患者结核分枝杆菌L型感染率调查(英文)

An Investigation on Infectious Rate of L-form of M. tuberculosis in Patients with Silico-tuberculosis

[目的]研究矽肺结核患者结核分枝杆菌L型的感染状况及其临床意义。[方法]对矽肺结核组与矽肺组患者进行痰结核分枝杆菌和结核分枝杆菌L型培养,并使用IK(intensifiedKinyoun)染色法对培养物作进一步鉴定。[结果]矽肺结核组126位患者中,结核分枝杆菌培养阳性18例,检出率为14％;结核分枝杆菌L型培养阳性64例,阳性率为51％,两者相比差异极其显著(P<0.001)。且矽肺期别越高,痰结核分枝杆菌L型的阳性检出率也越高(Ⅰ期为38％、Ⅱ期为67％、Ⅲ期为91％),各期患者间差异极其显著(P<0.001)。矽肺组102位患者中,结核分枝杆菌培养均为阴性,而结核分枝杆菌L型培养阳性8例,阳性率为8％。[结论]矽肺结核患者结核分枝杆菌L型的感染率较高,开展结核分枝杆菌L型检测对减少矽肺结核复发的漏诊或误诊,提高矽肺结核的早期诊断率方面有着重要的意义。

[Objective] To investigate the status of infection caused by L-form of M. tuberculosis in patients with silico-tuberculosis and evaluate its clinical significance.[Methods] Sputum culture for M. tuberculosis and its L-form was conducted in both groups of patients with silico-tuberculosis and silicosis, and IK(intensified Kinyoun) acid fast smear was used for further identification of infection with tuberculosis.[Results] In silico-tuberculosis group of total 126 cases, culture positive rate of M. tuberculosis was 14% (18 cases), and that of L-form of M. tuberculosis was 51%(64 cases) ,the diflerence was quite notable(P<0.001).Furthermore,the detected positive rate of L-form ascended following the increase of silicosis stage(38% was for stage Ⅰ ,67% for stage Ⅱ and 91% for stage Ⅲ), the striking difference among various stages was revealed(P<0.001).While in silicosis group, none of all 102 cases was found positive sputum M.tuberculosis culture,but positive culture rate of its L-form reached 8%(8 cases). [Conclusion] The infectious rate of L-form of M. tuberculosis in patients with silico-tuberculosis is much high,it is of important significance to carry out detecting L-form of M. tuberculosis,which can diminish misdiagnosis of silico-tuberculosis recurrence,and facilitate early diagnosis of silico-tuberculosis.
