桥小脑角脑膜瘤的显微外科手术治疗

Microsurgical treatment of cerebellopontine angle meningiomas

目的探讨桥小脑角脑膜瘤的临床分型及相应的手术治疗方案,提高显微外科手术治疗效果。方法回顾分析我院近5年来收治的18例桥小脑角脑膜瘤患者,根据术前影像学表现将桥小脑角区脑膜瘤分为四种类型,根据病例分型的不同采用不同手术入路。结果本组肿瘤全切率为88.9%,面神经和位听神经功能大部分保留。复发1例,无死亡病例。结论术前详细的检查和完善的手术方案制订对桥小脑角区脑膜瘤的手术治疗至关重要,显微外科手术是治疗桥小脑角区脑膜瘤安全有效的方法。

Objective To summarize the clinical characteristics , clinical classifications and surgical therapeutic regimen of cerebellopontine angle meningiomas(CPAM) and discuss microsurgical skills in operation procedure in order to achieve the tumor total resection and improve the protection of nerve functions and the brain stem. Methods 18 cases of CPAM treated in our department in the last 5 years were analyzed retrospectively. According to the image data, CPAM was classified into 4 types and corresponding surgical approaches were applied. Results Total resection of CPAM was achieved in 88.9%. The facial nerve and the vestibulocochlear nerve functions were preserved in the majority of all cases. No patient died and only 1 patient recurred. Conclusion Microsurgery is a safe and effective method for the treatment of CPAM. Careful examination before operation and perfect operative sheme are important for the treatment of CPAM.
