累及左颈总动脉起始部巨大真性动脉瘤的手术治疗

Radical operation of giant carotid artery aneurysm infiltrated beginning of common carotid artery

目的探讨累及颈总动脉起始部的巨大真性动脉瘤的手术方法。方法回顾性总结3例临床治疗资料。胸部正中切口显出前纵隔,切开心包及其返折显出心脏及其大血管,联合颈部切口显出动脉瘤全貌,升主动脉-颈内动脉自体静脉转流后,于主动脉弓上切除动脉瘤。结果3例均于术后9d治愈出院,分别随访3月、5a、6a无动脉瘤复发。结论利用自体静脉升主动脉颈内动脉移植技术,经胸部正中切口根治累及颈总动脉起始部的巨大动脉瘤,起始部显露清楚、操作方便,不切开胸腔、创伤小、恢复快,是一种较好的手术方案。

To investigate the radical operation of giant carotid artery aneurysm infiltrated the beginning of common carotid artery. To summary the clinic data of three patients. The carotid artery aneurysm were removed and the auto great saphenous veins were transplanted between ascending aorta and internal carotid artery through anterior mediastinum with midsternal incision road. Three patients were cured and were followed up 3 month, 5 years, and 6 years respectively without any recurrence signs. [Conclusion] It is a better operation to treat giant carotid artery aneurysm infiltrated the beginning of common carotid artery. by using of auto great saphenous vein bypass with midsternal incision road; which holding mini trauma, exposure clearly, quick recovery advantage without thoracotomy.
