膝关节负荷改变对关节软骨的影响 被引量:3

摘要 目的:观察一侧膝关节固定的条件下对侧膝关节软骨的影响。方法:6月龄,体质量2.5kg新西兰白兔10只,分实验组和对照组。实验组分a,b两部分,a组3只、b组5只。用石膏固定一侧膝关节5.5周。a组去除固定后即刻取对侧膝关节软骨标本。b组去除固定4周后取对侧膝关节软骨标本。通过光镜及扫描电镜观察膝关节负荷改变对关节软骨的影响。对照组2只,膝关节不做任何处理。结果:兔一侧膝关节固定5.5周,对侧膝关节由于承重增加,在去除固定后即刻,对侧膝关节软骨即出现退行性改变。去除固定4周,对侧膝关节软骨尚未恢复正常。结论:①一侧膝关节固定,对侧膝关节由于负荷的增加可导致退行性变化。②在相同时间段,关节固定对软骨的影响大于负荷改变对软骨的影响。 AIM: To observe the effect of unilateral fixation of knee joint on the joint cartilage of knee.METHODS:Ten 6 month old New Zealand white rabbits with a body mass of 2.5 kg were divided into experimental group and control group, and then the experimental group was subdivided into parts a and b, with 3 in group a and 5 in group b. The unilateral knee joint was fixed by plaster for 5.5 weeks. The sample of contralateral articular cartilage of knee was taken in group a immediately after the fixation was removed and in group b after four week fixation was removed. The effect of lode change of knee joint on the articular cartilage was observed under light microscope and scanning electron microscope. There were two rabbits in the control group, and their knee joints were not treated.RESULTS: The unilateral knee joint of rabbit was fixed for 5.5 weeks. Because the weight bearing of the contralateral knee joint increased, the regressive change of contralateral articular cartilage of knee occurred immediately after the fixation was removed.The contralateral articular cartilage of knee did not recover 4 weeks after the fixation was removed for 4 weeks.CONCLUSION: ① The regressive change is caused by the increase of contralateral knee joint load with unilateral knee joint fixed. ② In the same period, the effect of articular fixation on the cartilage is greater than that of load change on the cartilage.
