产超广谱β-内酰胺酶大肠埃希氏菌感染的监测

摘要 目的 了解临床病人感染大肠埃希氏菌超广谱 β 内酰胺酶 (ESBLs)产生情况及耐药性。 方法 采用表型确认实验检测ESBLs ,用Kirby Bauer琼脂扩散法作药敏实验 ;ESBLs阳性菌株进行 14种抗生素的药敏实验。结果  16 8株大肠埃希氏菌中 ,共检出产ESBLs菌 30株 ,检出阳性率为 17 9% ,以呼吸道和泌尿道感染为主。对青霉素类、头孢菌素类、喹诺酮类或氨曲南几乎全部耐药 ,对亚胺培南、阿莫西林 /克拉维酸敏感。结论 大肠埃希氏菌产ESBLs菌株对大多数抗生素耐药 ,故应重视ESBLs的检测。根据细菌耐药特性 ,合理使用抗生素。 Objective To investigate extended spectrum β lactamase(ESBLs) producingand resistance of Escherichia coli which clinical patientswere infected.Methods phenotypic onfirmmatory test was used to detectESBLs. Kirby bauer agar diffusion methods was used to judge drug sensitivity.Results 30 strains of Escherichia coli ESBLs(17 9%) were inspected in 168 strains Escherichia coli, in which infection was mainly in respiratory and urinarysystem. Escherichia coli ESBLs were resistant to penicilin, cephalospoorine and quinazolone, while they were sensitive to imipenem amoxicillin/clavulanate.Conclusion NS BeacauseEscherichia coli ESBLs were resistant to most of antibiotics, the surveillance of ESBLs should be performed. Antibiotics shoule be used reasonablly according to resistance of bacteria.
