长托宁对浅低温心不停跳心肺转流期间微循环功能的影响 被引量:33

摘要  目的 探讨长托宁对浅低温心不停跳心肺转流(CPB)期间微循环功能的影响。方法选择30例风湿性心脏病心内直视瓣膜置换术的病人,随机分为对照组(A组)和长托宁组(B组)。B组转机前在预充液中加入长托宁3 mg,A组不用长托宁。体外循环转机 10 min后开始分别观察两组病人的经皮脉搏血氧饱和度(SpO2)及动脉血氧饱和度(SaO2),转机后 30 min分别抽动脉血测乳酸(BL)含量及记录心率(HR)、平均动脉压(MAP),记录转机中尿量、中心温度(直肠温)复温时间。结果 B组BL较A组显著降低(P<0. 05);B组直肠温复温时间较A组显著减少(P<0. 05)。结论长托宁能较好地改善浅低温心不停跳体外循环期间微循环的功能。 Objective To investigate the effect of penehyclidine hydrochloride on microcirculation during cardiopulmonary bypass(CPB) with heart beating.Methods Thirty patients with rheumatic heart disease undergoing valve replacement were divided into group A(control group) and group B(penehyclidine group).In group B,3mg of penehyclidine hydrochloride was added to the priming solution.The parameters of mean arterial pressure(MAP),heart rate(HR),urinary output during CPB,time of temperature recovery,transcutaneous blood oxygen saturation(SpO_2),arterial blood oxygen saturation(SaO_2),and blood lactate were measured.Results Blood lactate and time of temperature recovery were decreased in group B as compared with those in group A(P<0.05).Urinary output and SpO_2 were significantly increased in group B than those in group A(P<0.05).Conclusion Penehyclidine hydrochloride can improve the function of microcircuration during open heart surgery under CPB with heart beating.
