面颈部猫抓病的临床表现和治疗

Clinical characteristics and treatment of patients with cat-scratch disease of maxillofacial and neck region

目的对面颈部猫抓病的临床表现、诊断和治疗进行探讨。方法对1997￣2002年治疗的25例面颈部猫抓病患者的临床症状和体征、病理表现及治疗进行比较分析。其中男性17例,女性8例;年龄2￣26岁。结果14例无明显临床症状,11例出现全身症状如发热等,5例累及多个淋巴结;手术15例;切开排脓5例其中有4例出现创面愈合速度慢。结论猫抓病诊断较困难,临床医师往往做不出猫抓病的诊断。该病手术效果较好,但切开排脓要慎重。

OBJECTIVE To study the characteristics, diagnosis and treatments of cat-scratch disease of ma- xillofacial and neck region. METHODS Clinical symptoms, physical signs, pathologic presentations, and therapeutic effects were analyzed in 25 patients with cat- scratch disease from 1997 to 2002. There were 17 males and 8 females, age ranged from 2 to 26 years old. R E S U L T S Most of the lesions (14 cases) were subclinical, 11 cases had constitutional symptoms (fever et al), 5 cases had complicated lymph nodes. 15 cases were undergone surgeries. 5 cases had incisions and drainages, 4 cases had delayed recovery of the epider- mal wound. CONCLUSION Cat-scratch disease is very difficult to diagnose. Clinician often miss the diagnosis. The effect of therapeutic resection was excellent. But incision and drainage should be cautiously used. [
