小切口白内障囊外摘除治疗硬核白内障疗效观察

Small incision ECCE and IOL to treat the hard nuclear cataract

目的 探讨小切口白内障囊外摘除联合人工晶状体植入术治疗硬核白内障的临床疗效。方法 对我院白内障患者 80例 (85只眼 ) ,通过 5 .0～ 5 .5 mm巩膜隧道切口、环形撕囊、旋转挽核及劈核等方法结合 ,进行白内障囊外摘除及人工晶状体植入术 ,对患者的视力、散光及并发症等资料进行观察分析总结。结果 术后 1天、1周、1月及 3月视力达到 0 .5以上分别占 91.7%、94 .9%、94 .4 %及 93.8% ,其中视力达到 1.0者分别占 15 .3%、36 .7%、5 9.7%、5 8.6 % ,术前散光和术后散光两两比较差异无统计学意义 ,其中 4只眼术中发生玻璃体脱出。结论 小巩膜隧道切口、环形撕囊、旋转挽核以及劈核等方法进行白内障囊外摘除 ,对处理硬核白内障、减少手术的并发症、提高临床疗效具有重要意义。

Objective To evaluate the clinical effect of small incision ECCE and IOL to treat the hard nuclear cataract.Method A retrospective study was underwent according to the 85 cases in our department from 2000.7 to 2003.1. Small scleral tunnel incision, circular tear capsulorhexis, rotating rolling nucleus and chopping nucleus were combined to do the extracapsular cataract extraction, ECCE and implantation of intraocular lens, IOL. The visual acuity, astigmatism and complications were summarize and analyzed.Results The visual rate of acuity more than 0.5 was 91.7%、94.9%、94.4% and 93.8% 1 day、1 week、1 month and 3 months after operation respectively. Among them, the visual rate of acuity more than 1.0 was 15.3%、36.7%、59.7%、58.6% respectively. The difference has not statistically significance before and after operation. Four cases have been arisen vitrieous outside.Conclusion The method is the combination of the scleral tunnel incision, circular tear capsulorhexis, rotating rolling nucleus and chopping nucleus. It benefit to improve clinical effect to treat the hard nucleus cataract and decrease the complication.
