芬太尼透皮贴剂治疗癌痛临床观察

Clinical Observation of Transdermal Fentanly in the Treatment Of Cancer Pain

目的 :观察芬太尼透皮贴剂 (商品名 :多瑞吉 )治疗中重度癌性疼痛的疗效及不良反应。方法 :以芬太尼透皮贴剂治疗 14 8例中重度癌痛患者 ,疼痛性质为慢性、持续性疼痛 ,初始剂量为 2 5 μg/h ,贴膜每 3日更换 1次 ,如 3天内出现暴发性疼痛用即释吗啡控制 ,3天后根据疼痛情况进行剂量调整。结果 :总缓解率为 97 6 % ,其中完全缓解 5 4 2 % ,明显缓解4 3 4 % ,中度缓解 2 7%。该药的不良反应较轻 ,主要有头晕、嗜睡、恶心、呕吐、便秘、口干和排尿困难等。结论 :芬太尼透皮贴剂治疗中重度癌痛疗效显著 ,有持久稳定的镇痛作用 ,使用方便 ,操用简单 ,不良反应小 。

Objective:To observe the treatment and side effects of Transdermal Fentanly(durogesic) on moderate and severe cancer pain.Methods:One hundred and forty-eight patients in moderate and severe cancer pain were treated with Transdermal Fentenyl.The beginning dosage was 25μg/h and the skin pad was changed every 3 days.If severe pain suddenly appeared during these 3 days,morphine should be used to control the pain.The dosage of the Transdermal Fentanly should be readdressed according to the level of pain after 3 days.Results:The total response rate was 97.6%,including a highly effective rate of 54.2%,an evident effective rate of 43.4%,and moderate effective rate of 2.7%.Side effects were slight—mainly constipations,sleepiness,nausea,and dizziness,lethargy,dry mouth,dysuria etc.Conclusion:Transdermal Fentanly can reduce moderate and severe cancer pain.It has a continuous and stable pain relief function,and it is convenient and easy to use.Side effects are slight,and it is adaptable those cancer patients who can’t take medicine orally or won’t follow the prescribed therapy time table.
