多西紫杉醇联合顺铂或卡铂治疗晚期非小细胞肺癌

Combination of docetaxel and cisplatin or carboplatin in the treatment of advanced non-small cell lung cancer

目的 :观察多西紫杉醇 (泰索帝 )联合顺铂或卡铂治疗晚期非小细胞肺癌的疗效及毒副反应。方法 :对病理学或细胞学证实的 32例晚期非小细胞肺癌患者给予泰索帝联合顺铂或卡铂治疗。采用每周疗法 ,泰索帝 2 5mg/m2 /次 ,静滴 ,第 1、8、15天三次给药 ;顺铂 75～ 80mg/m2 静滴 ,第 1～ 5天或第 2、9天分次给予 ;或卡铂AUC 6 0 7 0 ,静点 ,第 1天或第 2、9天分次给予。每 2 8天为一周期 ,每例患者均接受 2周期治疗或以上。结果 :全组 32例中获得完全缓解 1例 ,部分缓解 13例 ,稳定 16例 ,进展 2例 ,总有效率 4 3 7%。临床最常见的毒副反应为骨髓抑制 ,Ⅲ、Ⅳ度白细胞和血小板下降率分别为 15 6 %和 9 4 % ,其余无严重毒副反应。结论 :泰索帝联合顺铂或卡铂治疗晚期非小细胞肺癌疗效较好 ,毒副反应较轻 ,采用每周疗法 ,病人耐受性较好 。

Objective:To evaluate the effects and toxicities of the combination of docetaxel and cisplatin or carboplatin in the treatment of patients with advanced non-small cell lung cancer (NSCLC).Methods:Thirty-two patients with advanced NSCLC diagnosed by pathologic or cytologic examination were enrolled into the study.The patients received docetaxel 25 mg/(m 2·d) on day 1,8,15,and cisplatin 75～80 mg/m 2 on days divided into 1-5 or days 2,9,or carboplatin 6.0-7.0 on day 1 or days 2,9 of 28-day cycle.Each patient should at least complete two cycles.Results:Out of 32 evaluable patients,1 achieved complete response,13 achieved partial response,16 had stable disease,and 2 were progressive with an overall response rate of 43.7%.The main toxicities were leukopenia(15.6% in grade Ⅲ+Ⅳ) and thrombocytopenia(9.4% in grade Ⅲ+Ⅳ).Conclusion:Docetaxel combined with cisplatin/or carboplatin is effective and active.This per week scheme make it's toxicity slight and well tolerated especially for the old and advanced patients.
