L-OHP联合5-Fu和CF治疗晚期大肠癌的疗效观察

The Clinical Observation of Oxaliplatin with 5-fluorouracil,Leucovorin in the Treatment of Advanced Colorectal Cancer

目的 观察奥沙利铂联合 5 氟脲嘧啶、甲酰四氢叶酸钙治疗晚期大肠癌的近期疗效。方法 对 43例晚期大肠癌采用奥沙利铂 13 0mg/m2 静脉点滴 2h ,第 1天 ;甲酰四氢叶酸钙 2 0 0mg/m2 静脉点滴 2h ,第 1～ 5天 ;5 氟脲嘧啶 40 0mg/m2 静脉点滴 8h以上 ,第 1～ 5天。结果 全组CR 1例 ,PR 2 0例 ,总有效率为 48.8% (2 1/4 3 ) ,中位无进展生存期为 10 .1个月 ,中位生存期为 2 0 .7个月 ;主要不良反应为骨髓抑制、神经毒性反应 ,Ⅲ度白细胞减少发生率为 11.6% (5 /4 3 ) ,其它不良反应较轻。结论 奥沙利铂联合 5 氟脲嘧啶、甲酰四氢叶酸钙对晚期大肠癌有明显疗效 ,多数患者能耐受。

Objective To observe the clinical effect of Oxaliplatin with 5-fluorouracil,Leucovorin in the treatment of advanced colorectal cancer.Methods Forty-uhree patients with advanced colorectal cancer were selected and adopt Oxaliplation 130 mg/m2 intravenous drip 2 hours(the first day),Leucovorin 200 mg/m2 intravenous drip 2 hours(from the first day to the fifth day),5-fluorouracil 400 mg/m2 intravenous drip over 8 hours(from the first day to the fifth day).Results The total response rate of this group was 48.8%(21/43) including 1 CR and 20 PR.The median PFS was 10.1months and the median survival was 20.7 months.The main toxicities were Bone marrow depression and neurotoxicity,11.6%(5/43) patients developed Grade 3 WHO toxicity of leukopenia.other toxicities were mild.Conclusion Oxaliplatin with 5-fluorouracil,Leucovorin was obviously therapeutic effect in the treatment of advanced colorectal cancer.It could effectively extend the survival period.Most of patients could be tolerant.
