急性心肌梗死中胰岛素的作用

摘要 目的 :为探讨急性ST段抬高的心肌梗死用尿激酶 (UK)溶栓后应用胰岛素的作用。方法 :选取32例急性心梗患者均接受尿激酶溶栓后随机分成两组 ,其中一组静脉点滴葡萄糖、胰岛素及氯化钾 (GIK) ,另一组静点生理盐水和氯化钾 ,分别持续用 4 8h。在 4 8h内测定CK ,CKMB的变化及在 90min内测量抬高ST段下降百分比。结果 :在胰岛素组中CK及CKMB峰值提早且趋向于减低 ,特别是在前壁心梗的患者当中。结论 Objective:To explore the effect of insulin in patients with ST-elevation myocardial infarction(STEMI) after thrombolytic therapy by urokinase.Methods:32 patients receiving urokinase were randomized to receive intravenous glucose-insulin-potassium(GIK) or normal saline and potassium for 48 hours.The levels of CK and CKMB were measured in 48 hours.The percent of ST-descent was measured at 90 minutes.Results:The peaks of CK and CKMB reached earlier and tended to decrease in patients treated with insulin,especially in inferior MI.Conclusion:Insulin has an profibrinolytic effect in acute MI.
