青壮年无神经损伤胸腰椎爆裂骨折的手术治疗

Surgical Treatment of the Thoracolumbar Burst Fracture without Neural Impairment in Younger Patients

目的 探讨手术治疗青壮年无神经损伤胸腰椎爆裂骨折的临床价值。方法 对 30例 5 0岁以下成人无神经损伤胸腰椎爆裂骨折患者施行复位内固定 ,经伤椎椎弓根植骨同时后路椎板间植骨。结果 随访 4～ 12个月 ,功能恢复良好。影像学显示脊柱无后突畸形 ,恢复的椎体高度无下降 ,亦无腰背痛发生。结论 手术治疗青壮年无神经损伤胸腰椎爆裂骨折 ,能最大限度地恢复脊柱三维空间结构和生物力学状态 。

Objective To explore the clinical value of the surgical treatment of the thoracolumbar burst fracture without neural impairment in younger patients.Methods 30 patients whose age less than fifty years were treated by open reduction and internal fixation(ORIF).Interlaminar fusion and bone graft into fractured vertebral body through pedicles were performed at the same time.Results The follow-up period was from 4 to 12 months and good function was achieved in all cases.Thoracolumbar kyphosis was corrected after operation.Significant decrease has not been found during the follow-up period.Conclusion ORIF and bone graft is ideal treatment for thoracolumbar burst fracture without neural impairment in the younger,which can be effectively used to restore the 3-D construction and the biomechanical state of the vertebrae.It will effectively prevent the occurrence of the sequela of the thoracolumbar burst fracture.
