非小细胞肺癌癌周微淋巴管密度的检测及其临床意义 被引量:6

摘要 目的 :探讨非小细胞肺癌癌周微淋巴管密度 (L MVD)与其临床病理特征及预后的关系。方法 :选取 75例临床病理资料完整的非小细胞肺癌癌组织 ,应用免疫组化双染试剂盒及 FV- - RA和 型胶原单克隆抗体在同一张切片上分别对内皮细胞和基膜进行标记 ,在光镜下观察微淋巴管密度。结果 :1非小细胞肺癌癌周组织中 L MVD为(14 .73± 3.5 7)显著高于肺良性病变组织 (7.5 8± 4 .2 9)。2微淋巴管密度与肿瘤组织原发肿瘤的大小、淋巴结的转移、P- TNM分期及细胞分化程度等均有密切相关 (P<0 .0 1) ,而与发病年龄、性别、肿瘤的原发部位 ,组织类型无关。 3肺癌组织中微淋巴管高密度组患者 5 a生存率明显低于低密度组 ;与肺癌患者术后生存期显著负相关。结论 :微淋巴管密度在肺癌的发生、发展、转移中起重要作用 ,可作为非小细胞肺癌生物学行为的一项评估指标 ,检测非小细胞肺癌中L MVD有助于预测非小细胞肺癌的预后及指导术后综合治疗。 Objective To investigate the relationship between lymphatic microvessel denity and clinical pathological features and prognosis in patients with non-small lung cancer.Methods The data of 75 cases of non-small cell lung cancer were collected.Anti-FVⅢ RA monocolonal antibody and anti-collagen IV monocolonal antibody were used to mark endotheliocyte and basament membrane by double immunohistochemical staining.Then lymphatic microvessel density were observed under microscope.Results LMVD in non-small cell lung cancer was (14.73±3.57);That was significantly higher than that in benign pulmonary tissues(7.58±4.29).The LMVD was closely related to the size, differentiation degree, P-TNM stages,lymph node metastasis.but not to age,sex,histological classification and site of the cancer.The 5-year survival rate in patients with high LMVD was significantly lower than that in patients with low LMVD; The survival time of patients with non-small cell lung cancer was negatively related to LMVD.Conclusion Increase of LMVD may play an important role in the oncogenesis,development, metastasis of non-small cell lung cancer and might be served as a tumour marker to evaluate the biological behavior of non-small cell lung cancer. Detection of LMVD might be helpful to predit prognosis and the postoperative multi-modalitytherapy in patients with non-small cell lung cancer.
