狼疮性肾炎病变肾组织中bFGF、CD68和PCNA表达的研究

摘要 目的 通过对狼疮性肾炎 (LN)与肾小球轻微病变 (GML)肾组织中bFGF、CD6 8、PCNA的原位检测和相关分析 ,以了解它们在LN发病中的意义及其相互关系。方法 以 14例GML为对照组 ,采用ABC免疫组化法对 2 3例LN病例的肾穿刺组织 ,分别检测其bFGF、CD6 8和PCNA表达情况。结果 与GML病例相比 ,LN组肾小球bFGF表达明显增强 (P <0 0 5 ) ,CD6 8和PCNA表达的阳性细胞数也明显增多 (P <0 0 1) ;IV型LN组肾小球bFGF明显强于II型LN组 ;LN组肾小球内CD6 8+ 与PCNA+ 细胞数间呈正相关 (r =0 5 6 ,P <0 0 5 ) ,其中IV型LN组中有肾小球新月体形成的病例 ,CD6 8+ 与PCNA+ 细胞数间也呈正相关 (r =0 86 ,P <0 0 5 )。结论 LN病变肾小球bFGF、CD6 8和PCNA表达均明显增强 ,其表达在某种程度上可反映临床LN病变程度的严重性。 Objective To study the expressions of bFGF, CD68 and PCNA in diseased renal tissues from patients with lupus nephritis (LN) and glomerular minimal lesion (GML) by immunohistochemistry, and to observe their significance and correlation in thedevelopment of LN. Method The expressions of bFGF, CD68 and PCNA in renal biopsy tissues from 23 patients with LN and 14 patients with GML were detected separately by ABC immunohistochemical staining. Result The expression of bFGF in glomeruli was remarkably enhanced (P<0.05), and the number of CD68 +and PCNA +cells in glomeruli also significantly increased (P<0.01) in LN when compared with those of GML. The expression of bFGF in glomeruli of the type Ⅳ LN group were stronger than that of the type Ⅱ LN group The number of PCNA positive cells was related with that of CD68 positive cells in diseased glomeruli of LN(r0.56,P<0.05), and the same condition was identified in diseased glomeruli with crescent formation from 8 patients of the type Ⅳ LN group (r0.86, P<0.05).Conclusion The increased expressions of bFGF , CD68 and PCNA in diseased glomeruli from patients with LN was related to the clinical severity of LN.
