The recent studies have suggested that there are close correlations between the hemorrhagic transformation caused by recombinant tissue plasmonigen activator (rtPA) and matrix metalloproteinases-9 (MMP-9). The increase of the levels of MMP-9 both in blood and cerebrospinal fluid predicts subsequent hemorrhagic transformation in cerebral infarction. MMP-9 inhibitors are expected to reduce the incidence of rtPA-induced hemorrhagic transformation in cerebral infarction. Blood-brain barrier disruption and other changes on enhanced magnetic resonance imaging and diffusion-weighted imaging may predict the hemorrhagic transformation.
