心电图记分法对肺心病急发期的诊断价值

Value of electrocardiogram synthesize score index on diagnosing acute exacerbation of chronic pulmonary heart disease.

目的 探讨心电图综合记分指标对肺心病急发期的诊断价值。方法 对 10 0例肺心病急发期患者的心电图与 80例肺心病缓解期患者的心电图进行对比 ,依次分析肺心病急发期患者心电图出现率较高的单项指标 :肺型P波、QRS波电轴右偏或胸导顺钟向转位、窦性心动过速、肢导低电压、STⅡ、Ⅲ、AVF压低、房性心律失常、室性心律失常 ,将上述单项指标中的肺型P波记 2分 ,余单项指标各记 1分 ,以综合记分指标≥ 4分、≥ 5分的心电图诊断肺心病急发期的指标进行比较 ,并与肺心病缓解期心电图结果对照。结果 心电图综合记分指标≥ 4分诊断肺心病急发期的敏感性、特异性、准确性、阳性预测值、阴性预测值分别为 99%、72 5 %、87%、81 8%、98%。而≥ 5分诊断肺心病急发期的敏感性、特异性、准确性、阳性预测值、阴性预测值分别为 76%、97 5 %、85 6%、97 4%、76 5 %。结论 心电图综合记分指标≥ 5分比≥

Objectiv To investigate the value of electrocardiogram (ECG) synthesize score index on diagnosing chronic pulmonary heart disease (CPHD).Methods Make a comparison between the ECG of 100 patients with acute exacerbation of CPHD and ECG of 80 patients with recovery of CPHD P pulmonary,QRS wave right axis deviation or chest lead clockwise rotation,sinus tachycardia,Limb Lead Low voltage,ST segment depresed in lead Ⅱ,Ⅲ,AVF,atrial arrhythmias,ventricular arrhythmias of frequency of occurrence higher were analysised in the ECG of patients with acute exacerbation of CPHD.P pulmonary in the forementioned alone index score 2,the remaining each alone index score 1,Comparison between ≥4 and≥5 with the synthesize score index of ECG in diagnosing acute exacerbation of CPHD,the diagnostic criteria again compare with control group.Results The susceptibility,specific,accuracy,positive predicting value,negative predicting value of ECG synthesize score index≥4 in diagnosing acute exacerbation of CPHD was 99%,72.5%,87%,81.8%,98% respectively,and the susceptibility,specific accuracy,positive predicting value,negative predicting value of ECG synthesize score index≥5 in diagnosing acute exacerbation of CPHD was 76%,97.5%,85.6%,97.4%,76.5% respectively.Conclusion The value of ECG synthesize score index of ECG synthesize score index≥ 5 was better than ≥4 on diagnosing acute exacerbation of CPHD.
