摘要
目的 通过对中期妊娠的大脑前叶径(CCD)、小脑横径(TCD)及两者之比的数据分析,了解其在不同孕周的正常值及异常妊娠时的变化,为早期胎儿发育异常提供超声诊断依据。方法 正常妊娠孕18~30周间孕妇,分13组共416例,异常妊娠5例(2例IUGR,1例重度脑积水,1例21三体,1例四肢短小),测量双顶径(BPD),CCD,TCD及CCD/TCD比值。 结果 正常妊娠时CCD、TCD与BPD明显相关。随着孕周增加,CCD、TCD分别呈线性增长,CCD/TCD之比在整个妊娠期中约等于1。当胎儿发育异常时,CCD/TCD之比也发生异常变化。 结论 正常妊娠时CCD/ICD之比为1,当二者之比发生明显变化时,应高度怀疑异常的脑发育,尤其隐性的脑部异常或染色体异常。
Objective To establish the relationship between gestational age (GA) and cephalic cerebral diameter (CCD) or transverse cerebellum diameter (TCD) , and to assess the ratio of CCD / TCD in normal and abnormal pregnancy. Methods Totally, 416 normal fetuses of uncomplicated pregnancies between 18-30 weeks in 13 groups and 5 abnormal pregnancies (2 fetal growth restriction, 1 serve hydro-cephalus, 1 Down's syndrome and lachondroplasia). BPD,CCD, TCD and CCD/TCD were measured respectively. Results CCD and TCD of Normal fetuses were correlated well with the biparietal diameter (BPD). CCD and TCD increased linearly with the pregnancy weeks and their ratio closed to 1 in normal fetuses. When fetuses developed abnormal, the ratio of CCD/TCD changed. Conclusion In the normal fetuses, CCD / TCD is close to 1. When the ratio changed obviously, abnormal brain development may exist, especially for recessive abnormity of brain or heterotropic chromosome.
