心内直视手术不同术式肺功能变化

Changes of pulmonary ventilatory function between hemisternotomy incision and minimally open heart surgery

目的 :探讨不同术式心内直视手术期间肺通气功能的变化。方法 :对 30例择期心脏手术患者随机分为纵断胸骨组和肋间小切口两组 ,在麻醉手术期间及体外循环前后 ,采用旁气流呼吸监测方法测定通气功能的变化。结果 :Ppeak在停体外循环 (CPB)后 10min始增高 (P <0 .0 1) ;胸肺顺应性在开胸后 5min开始下降 (P <0 .0 5 ) ,手术结束时恢复至基础值水平 ,两组间比较有统计学意义。结论

Objective:To observe the changes of the pulmonary ventilation function during open heart operation with different incision under cardiopulmonary bypass(CPB).Method:Thirty patients with artrial or ventricular septal defects were randomly allocated to 2 groups receiving:group 1 hemisternotomy incision.group 2:minimally invasive cardiac surgery.The pulmonary function was measured with side stream spirometer.Result:P peak had significant changes 10 min after CPB ( P <0.01),The Total lung compliance increased 5 min before CPB( P <0.05),but returned to the baseline after the closure of insicion.Conclusion:The pulmonary ventilatory function of two groups is both reduced to some degree after CPB and cardiac operation.
