腹腔镜直肠癌Miles根治术12例的体会

Analysis of12cases with laparoscopic Miles’rectomy for rectal

目的 :探讨经腹腔镜直肠癌Miles根治术的临床应用。方法 :回顾分析 12例低位直肠癌 ,利用腹腔镜超声刀技术行直肠全系膜切除 (totalmesorectalexcision ,TME)Miles根治术 ,并对手术操作、术后恢复等情况进行临床分析。结果 :12例患者均顺利完成手术 ,手术时间 110～ 16 0分钟 ,平均 12 5分钟 ,术中出血 80～ 16 0ml,平均 12 0ml ;12例直肠系膜均完整 ;术后 2～ 3d恢复胃肠功能并下床活动 ,住院时间 7～ 9d ,平均8 2d。无因手术及术后并发症而死亡的病例。结论 :用腹腔镜方法行低位直肠癌Miles根治术 ,不仅可达到开腹根治术的目的 ,而且具备操作安全、微创、恢复快等优点。

Objective:To investigate the methods of laparoscopic Miles’rectomy for rectal cancer.Methods:Lap-aroscopic Miles’rectomy with ultrasonically activated scalpel were performed in12patients with low rectal cancer.Opera-tive procedures,complications and postoperative recovery were studied.Results:12patients were performed successfully.The operative time was125min(110-160)and operative blood loss was120ml(80-160).Gastric function recovered in all patients within2-3days and the average time in hospital was8.2days(7-9days).None of the patients died due to the operation and complications.Conclusions:Laparoscopic Miles’rectomy with TME is valid for low rectal cancer and is well characterized for its safety,minimal invasion and quick recovery.
