计算机影像导航引导技术在脊柱内植手术中的应用 被引量:2

摘要 目的 探讨运用计算机影像导航技术在脊柱手术中的应用范围和临床意义。方法 对 12 0例脊柱患者采用计算机影像导航引导下的脊柱手术 ,其中采用X线虚拟导航引导模式 10 5例 ,包括引导腰椎椎弓钉加椎间钛笼植入 4 0例 ,胸腰段椎弓钉植入 10例 ,颈椎内固定植入 32例 ;采用CT三维重建虚拟导航引导模式 15例 ,包括颈椎后路螺钉植入 5例 ,中上段胸椎弓钉植入 6例 ,脊柱侧弯多节段胸腰椎弓钉植入 4例。内植物完成 ,术中C臂透视摄片以了解内固定位置情况 ,术后CT进行椎弓根层面扫描。结果 腰椎组 :椎弓钉进钉点偏差不超过 1mm ,角度偏差不超过 2 °,实际内植物与虚拟内植物重叠率平均 96 %。术后CT椎弓根位置扫描 :94 % (2 70 /2 88)腰、胸椎弓根内中心位 ,18枚位于椎弓根内偏外侧 ,临床无神经症状。结论 导航技术对同时需要多路径显示 ,多维定位要求的脊柱手术创造了理想新颖的引导模式。术者的操作在无辐射条件下具形象化、多维化 ,且实时感强 ,使内植物按设想路径精确制导。
